3D FAITH DIABETES PREVENTION STUDY
Out of 88 counties in Ohio, Toledo ranks #11 in type 2 diabetes prevalence. Our project team is working with community members and organizations to design a customized version of the national Type 2 Diabetes Prevention Program for local faith communities. We will begin the program in 2026 and would like to partner with faith communities interested in hosting the program for their members. Please CLICK HERE to contact us.

DIABETES PREVENTION PROGRAM X TOPS STUDY
TOPS is an affordable, community-led weight loss and weight maintenance program. TOPS has shown tremendous success in helping people achieve healthy weight. This project aims to introduce TOPS after completion of the Culture of Health or Diabetes Prevention Program. Contact us of you are interested in this program.


CULTURE OF HEALTH TOLEDO
Many communities have a variety of assets and opportunities to promote health and wellbeing. However, churches that are interested in the health and wellbeing of their clergy and members may not have time to coordinate and plan a health promotion program. We want to create a city-wide culture of health by supporting faith communities to start a customized health program using existing community assets.
